Latest progress and achievements on Integrated Care initiatives

The latest progress and achievements within the Integrated Care Initiatives include:

  • three Auckland region DHBs are testing new ways of managing the care of people with long-term health conditions. The pilots involve eight GP practices in greater Auckland, several hospital speciality services, pharmacies and community care services
  • two trial sites have been identified for the Shared Maternity Record of Care, and the first site is expected to go live in April 2012
  • the eShared Care Record View is live at a small number of general practices and pharmacy sites and is in the process of being rolled out to all general practices in Christchurch
  • about 120 general practices have GP2GP functionality installed and 65 GP practices are using it already. Over the next 12 months, all GPs will be able to make this service available to patients who are moving location.
